My System of Care is changing that in the United States. We build personalized healing journeys for youth ages 12–26 who are navigating the transition out of a mental health crisis - co-designed with the young people, families, and communities at the center of care.
One of the hardest parts of healing starts after discharge.
Hospital staff do extraordinary work stabilizing youth in crisis. But, there is often no bridge between inpatient care, the community, and the discharge home. Local supports, peer connections, and ongoing coordination help make recovery last – that’s the gap we’re built to fill.

Our approach is adapted from Resource Groups, an evidence-based, patient-centered model shown to significantly improve empowerment, quality of life, and personal recovery.¹

We bridge hospital and home.
Located inside hospitals and residential facilities, My System of Care Hubs are where youth and their families take ownership of what comes next. Before discharge, we co-design an 8 to 12-week healing journey anchored in their local community - with evidence-based partners who are ready and waiting. Then, we stay with them through the journey to lasting recovery.
