Who is Eligible for System of Care Wraparound Services?
System of Care Wraparound Services are the highest level of community-based care available in Orange County, and we want to make sure that wraparound and care coordination services are provided to the youth and families who need them most. A child or young adult, ages 5-21, who is a resident of Orange County, and meets all three of the following criteria, is eligible for System of Care. This includes Young Adults, ages 18-21, who have previously accessed services only through the adult service system. Youth in residential care outside of Orange County, who will be returning to Orange County, are also eligible for System of Care services.
Eligibility Requires Meeting All Three of the Following Criteria:
1. Diagnosis of Serious Emotional Disturbance (SED) The identified youth/young adult must have an AXIS I diagnosis as defined by the Diagnostic and Statistical Manual (DSM) IV-TR and a Global Assessment Functioning (GAF) score of 50 or below.
2. Involved with two or more systems Department of Social Services, Probation, Mental Health, Schools.
3. AND Current challenges in successful living in the community, reflected by:
History of Psychiatric Hospitalization. Recent admission, extended stay, e.g. 2 in the past 2 years.
Intake requires a valid diagnosis, but we know that sometimes there are barriers that make it difficult to secure one. Please be assured that we will not turn any child or youth in crisis away because of a lack of diagnosis. System of Care Intake will work with referral sources, and can provide resources, to help youth who meet all of the above criteria but do not have a current diagnosis due to transportation, access or insurance issues.
Getting Started: How does this work?
Referral is reviewed & assessed for eligibility by a team of intake workers from Orange County Department of Mental Health, Probation, and Social Services, along with Family Partners from Family Empowerment Council, Inc. Intake workers will contact the referral source to gather information and complete the intake. If the youth/young adult is not eligible for SOC, the intake worker will direct them to other resources that may be helpful.
Eligible youth/young adult is assigned a Family Partner/Transition Coordinator. The assigned person meets with the youth/young adult/family at a location that’s convenient for them and helps prepare them for the Wraparound Planning Process. They provide assistance with identifying strengths, values, and preferences; deciding which life domains will be planned for; selecting the Wraparound Team; and explaining how the planning process works overall.
The youth/young adult/family decide when to schedule their first Wraparound Team meeting and identify who they want on their Team. Teams can include family members, friends, service providers, community supports, etc.
A trained Wraparound Facilitator conducts your first Team planning meeting and they are responsible for keeping the Team focused on the family's goals.
A safety/crisis plan is developed at the first meeting and a Care Coordinator is assigned to manage the plan. The youth/young adult/family’s needs drive which services will be provided and are tailored specifically to them.
Referrals are made to the appropriate services based on needs and assessments for level of care.
Wraparound Team meetings are held on a regular basis. These are an ongoing partnership with Team members, and everyone shares responsibility for the results.
Everyone participates in a follow-up evaluation to review the plan and its progress. The feedback is used to identify what parts of the plan worked well and which may need improvement.

Transition Coordinators help young adults:
Build a support group of people who “get them.”
Identify their unique strengths, special interests, and accomplishments.
Develop leadership abilities and increase their sense of identity, belonging and self-esteem.
Become an equal partner in the process to create one coordinated, individualized plan with their team of supporters.
